Direct Support Professional

Job Summary:
A Direct Support Professional provides support and care for individuals with Intellectual
Developmental Disabilities (I/DD) in a home or facility setting. This person will attend to daily
living activities and maintain open communication with parents or guardians and the Program
Manager to ensure that the people served are living their lives to their fullest potential. Acejel
Home Care provides the people supported with an opportunity for quality care in a faith-based
setting.

Essential Duties:
Description
 Provides support to people served by assisting in the development and implementation of
all support plans, including spiritual life support, assisting people in achieving their
personal goals and desires.
 Providing interaction and choices of activities that support a meaningful day and
supporting people to be independent in their daily living activities.
 Ensures client safety and maintaining a safe environment.
 Maintains open communication and cooperative work effort with other staff to ensure
quality and continuity of support to people served.
 Reminds and assists individuals with self-administration of medications with proper
training.
 Reports changes in individuals’ condition or family situation to administrators and
supervisors.
 Documents services provided include completing daily logs, incident reports, outcome
data sheets, in a concise, understandable, and legible manner.
 Assists people with fiscal management while safeguarding their funds. They may include
completing ledgers, reconciling accounts, and assisting people with spending choices.
 Plans meals, shopping for groceries, preparing and serving food/meals and clean-up.
 Accompanies individuals to scheduled appointments and transports them or running
errands for them.
 Ensures that people exercise their rights and that rights are not restricted without due
process, ensures people are treated with dignity and respect.
 Performs other duties as assigned.
