Responsibilities:
Client-Centered Care: Work directly with clients in their homes, assisting with daily living activities, personal care, and community integration. This may include; ambulating, grooming, toileting, dressing, eating, or other necessary daily functions.
Creativity and Independence: Use your creativity to address individual needs and empower clients to achieve their goals.
Clear Communication: Collaborate with team members, clients, and their families to ensure effective care coordination.
Empathy and Compassion: Understand the unique challenges faced by individuals with I/DD and provide compassionate support.
Problem-Solving: Be proactive in identifying solutions and adapting to changing circumstances.
